It appears that we have two concepts for the qualifier ANATOMIC LOCATION PROPERTY: 550001000004103 in Nebraska Lexicon as a subtype of LOCATION PROPERTY and 758637006 in SI. We need to agree on resolution of the duplication.

Are you using the Location (property) concept in your extension to define concepts?
If 758637006 |Anatomical location (property) (qualifier value)| is a duplicate of your extension concept, then Farzaneh Ashrafi and I think your extension concept should be inactivated and you should use the concept from the International Release. We recall this being discussed and agreed in the past, though we are not sure where the notes from those discussions are.

this was discussed in London 2017. We needed two separate property concepts to e.g. separate the two "measurement location" observables where one could have the answer "left upper arm" and the other "at home".
Now we have 758638001 | Geographical location (property) (qualifier value) | and 758637006 | Anatomical location (property) (qualifier value) | but no "location" common subsumer.

My bad that I did not followup and correct our extension. I will do so, substituting the International release concepts. We also created 530001000004102|Radial direction property(qualifier)| IS_A 118598001|Measurement property(qualifier)| for specifying the observations which are based on clock-face orientation describing the location of breast cancers and colon cancers. I do not see any such in SNOMED International, is that correct?

I agree and we need to review different use cases. We (CIMI) has "tunneling clock direction" and "undermining clock direction" coming from the wound. The CIMI cancer interoperability folks have a clock direction from the breast nipple to the tumor. So...these may be different or maybe we can model them using same concepts.

Tentatively, I believe that the same 530001000004102|Radial direction property(qualifier)| concept would be used to define "tunneling clock direction" and "undermining clock direction" as well as "breast tumor location" observables. Would be interesting to look at the specifics though. For "breast tumor location" the reference direction12 o clock is up/superior but what is the reference direction for wound undermining and tunneling?

James R. Campbell, we agree that the International Release currently does not have a concept for Radial direction property (qualifier) and agree that it should be submitted. You can submit the request to us via NLM so it can be brought into the International Release retaining your extension concept id.

I think I need a clarification here. We have stated intention for SNOMED International to deploy our pathology module to the International release. Will that not include Qualifiers, Body structures and related content that we have developed to support the logical definition of the pathology and genomic concepts necessary for CAP Cancer Protocols? 530001000004102|Radial direction property(qualifier)| is a necessary value for definition of breast and colon cancer concepts. I understand from Jim Case that material promoted from an extension RETAINS the SNOMEDCT ConceptID as it migrates to the international release

Yes, when (not if...) the "pathology module" is promoted that would necessarily include any concepts used in those definitions. The concept IDs are kept but the moduleIds are changed from "Nebraska Pathology Module" to "SNOMED CT core module"

Doing a quick review of the FSNs within the pathology module, I find Body structures, Findings, Observable entities, Situations and Qualifiers in numbers as listed below. Some of the properties need inactivation per previous discussion but I see a number of concepts that have not been promoted to International release

About a year ago I spoke with Jim Case regarding the use of precoordinated SNOMED CT in the pathology build and he gave me verbal approval to employ the Observables concept model to further define the concepts where the meaning seemed clear so that we would not have to create duplicate concepts for what already appeared in SNOMED CT. Therefore there are multiple observables in the International release, primarily 'grouper' concepts that we have defined, sometimes with Nebraska Lexicon attributes or values. For example:
371441004|Histologic type of tumor|(defined) isa Tumor observable Inheres in = Proliferative mass and Property = Histologic cell type
These concepts are in the core module but the application of the concept model for definition is work that has come from Nebraska. I assume that the requesting NRAs should also specify definitional work for precoordinated Observables that are in the realm of anatomic, molecular pathology and genomics.
