Diagnostic imaging terming guidanceJCA

A number of issues remaining on naming for diagnostic imaging procedures (IHTSDO-895). These issues were initially addressed by the IHTSDO Content Committee in 2012, but not fully resolved. The elaboration document for this tracker item contained the status of the issues. As this is a heavily used part of the terminology it is important to get the terming correct and to decide what is to be done with existing terms that do not conform.

  1. Fluoroscopy imaging - the content committee (2012) agreed that the use of near synonyms was acceptable for these procedures (i.e. angiogram/arteriogram). The committee also agreed that the word fluoroscopic was unnecessary in any description other than the FSN. At that time KSP did not agree that guidance is reliable and needed radiologist input before sign-off.
  2. Multiple modality procedures - Should the abbreviation be the PT instead of a synonym? The preference for translation purposes is for clarity, i.e. the most descriptive non-FSN. The clinical preference in English is likely to be shorter, less clear for translation, and include abbreviations. There needs to be more stability for PT assignment. This needs further thought. All items with abbreviations need to be revisited.
  3. View/Projection/Technique adjustments - Pre-coordinated equivalents cannot be modeled and currently are not being added to SNOMED CT. Association of a projection/technique would be handled by other mechanisms than SNOMED CT. Is this reasonable? Needs further discussion.
  4. Should existing DI terms be revised to conform to the naming patterns described in the Elaboration document

Normative relationship grouping editorial guidance

Initial proposed grouping guidance has been added to the SNOMED International Editorial Guide for four hierarchies: Clinical findings (disease only), Procedures, Situation with explicit context, and Observable entity. As we begin developing concept models for additional top level hierarchies, there is a need for additional guidance on how to create relationship groups with approved attributes.

Discussion on this topic was not covered as it was in a timing conflict with the MAG.

 
10Clarification on "substantive change"JCA

When trying to apply quality improvement process, there are often a large number of changes applied to existing content. While the current policy refers to changes related to the FSN, there are other types of changes to content that may be construed as substantive. 7.3.1 Changes in the Fully Specified Name

Examples include:

  • Changes to modeling that alters the transitive closure
  • Batch changes to FSN terms to adhere to naming patterns
  • Bulk movement from one top level hierarchy to another (e.g. clinical finding to situation)

SNOMED Int'l needs to develop a more robust policy related to what is a substantive change and what is the notfication/approval process needed to be completed before the change can be released.
