The Patient Care Workgroup in HL7 has over the past 3 years assembled a database of over 100 million allergy list entries from several US sources: VA, Kaiser, Cerner, Optum.   One of the things we found was that less than 300 drugs account for ~95 % of the entries.

Review of allergen classes and questionable allergen groupers in allergy to substance hierarchy

I am posting 2 files created by the allergy update project for review by our group. The first file contains a list of allergen/pseudoallergen classes. We wish to solicit input from this group as to whether these need to be retained in the revisions to the substance hierarchy. Note that the use of allergen classes (as opposed to just substances) has been used only infrequently for the modeling of allergies. The second file lists all of the descendants of allergic disposition and some questionable groupers which we need to determine clinical relevance for in order to guide the modeling of the causative agents in the substance hierachy

Updates to model of allergic disease

  • Treat allergic diseases as pathological structures and model with a role group containing: associated morphology+finding site+causative agent+pathological process=allergic process
  • Approval to initially add additional allergic processes of IgE-mediated allergic process, non-IgE mediated allergic process and contact hypersensitivity process
  • Bruce Goldberg to test addition of additional pathological processes corresponding to Gell and Coomb's hypersensitivity mechanisms in order to full define allergic disorders by mechanism
