Discussion/Actions

1

Welcome and role call
BGO, all

2

Follow-up of issues raised by Marie-Alexandra Lambot

BGO
All issue entered into CRS

Discussion centered around whether Pseudoallergy to sulfite food preservatives was required as the creation of a structural + role based grouper is not likely to be supported. It was noted that sulfites are are already under food preservative in SNOMED CT. A question arose as to whether not only are sulfites preservatives for non-food substances but whether pseudoallergic reactions to these have been reported. Marie-Alexandra Lambot will research this further with her allergy colleagues. The group on the whole felt that Pseudoallergy to sulfite would suffice.

3

Buy-in from group for proposed changes to allergy conceptmodel
Review of existing model and proposed changes
Briefing paper for end users

BGO

Recommendation from Jim Case and ECE project group to obtain buy-in from this CRG in order to expedite proposed changes

  • Review of existing model and proposed changes
  • Briefing paper for end users
  • Group generally agreed with proposed changes to concept model with the possible exception of moving the condition and disposition classes under clinical findings
  • Bruce Goldberg will discuss further with head of terminology

4

Nonallergic hypersensitivity/pseudoallergy/ intolerance

BGO

Feedback from ECE project group

  • Should pseudoallergy be retained as the FSN with nonallergic hypersensitivity added as a synonym or PT?
  • Should the existing FSN be changed to Nonallergic hypersensitivity and pseudoallergy retained as a synonym (or PT?)

Creation of specific pathological processes to represent different nonallergic hypersensitivity mechanisms

  • Given that most existing content under pseudoallergy represents unknown pathologic processes, the creation of more granular pathological processes as proposed in order to define pseudoallergy concepts based on underlying mechanism was thought not to be warranted at this time

Should definition of non-allergic hypersensitivity include ...symptoms resembling allergy....?

  • Discussion centered around definitions and relationship of Nonallergic hypersensitivity, pseudoallergy and intolerance:
    • Is pseudoallergy really obsolete or is it a more specific type of nonallergic hypersensitivity?
    • Are intolerance and nonallergic hypersensitivity the same or is intolerance a more general grouper encompassing both allergy and pseudoallery or is intolerance a subtype of hypersensitivity or nonallergic hypersensitivity?
  • Bruce Goldberg to prepare diagrams illustration possible hierarchical relations of the above concepts for review during the next call
